Q: Is this a dental insurance policy?

No. This is an in-office dental savings plan — not an insurance policy, PPO, HMO, or state-licensed program. It is a direct savings agreement between our practice and our patients.

Q: Where is this plan valid, and can it be transferred?

The plan is valid only at Modern Dental Concepts in Warren, OH. It is non-transferable — benefits are for the enrolled individual only and cannot be used at any other dental office.

Q: What is the plan's duration, and do unused benefits roll over?

The plan is valid for 12 months from the enrollment date. Unused benefits (cleanings, exams, fluoride) do not roll over — they expire at the end of the plan year. Plans auto-renew annually unless cancelled 30 days before the renewal date.

Q: Can I combine this plan with dental insurance?

No. This plan is designed for uninsured patients only. The member discount cannot be combined with dental insurance benefits, Medicaid, or any other discount program.

Q: Is the fee refundable if I don't use the plan?

No. The annual fee is non-refundable once enrolled, even if benefits are not used within the plan year. The annual fee is due in full at the time of enrollment.

Q: Does the discount apply if I'm referred to a specialist?

No. When referred to an outside specialist, the savings plan discount does not apply at the specialist's office. The discount is valid only at Modern Dental Concepts.

Q: Are there any treatment exclusions?

Yes. The 10% member discount does not apply to implant services or orthodontic treatment. Ask our team about our in-house payment plan options available for orthodontic patients.
