Plan Terms & Conditions

No. This is an in-office dental savings plan — not an insurance policy, PPO, HMO, or state-licensed program. It is a direct savings agreement between our practice and our patients.

The plan is valid only at Modern Dental Concepts in Warren, OH. It is non-transferable — benefits are for the enrolled individual only and cannot be used at any other dental office.

The plan is valid for 12 months from the enrollment date. Unused benefits (cleanings, exams, fluoride) do not roll over — they expire at the end of the plan year. Plans auto-renew annually unless cancelled 30 days before the renewal date.

No. This plan is designed for uninsured patients only. The member discount cannot be combined with dental insurance benefits, Medicaid, or any other discount program.

No. The annual fee is non-refundable once enrolled, even if benefits are not used within the plan year. The annual fee is due in full at the time of enrollment.

No. When referred to an outside specialist, the savings plan discount does not apply at the specialist's office. The discount is valid only at Modern Dental Concepts.

Yes. The discount applies only to treatment performed in our office. It does not apply to implant services or orthodontic treatment, and it does not apply to anything we refer out — molar root canals, impacted wisdom teeth and IV sedation cases are all handled by a specialist, so no member discount applies to them. Simple and surgical extractions, erupted wisdom teeth, and anterior and premolar root canals are done here and do qualify. Ask our team about our in-house payment plan options available for orthodontic patients.
